DRIVER’S TRAINING LOG

DMV Driver’s Training Log is now MANDATORY.

Any member preparing for the DMV Class B drive test shall maintain this form. The completed form shall be brought with you and given to the Driving Coordinator prior to the drive test. A blank form can be printed by clicking the link below.

LICENSE CLARIFICATION

There has been some confusion as to what the proper driver license class and endorsements are. Members holding the ranks of Firefighter III through Captain II shall maintain a driver’s license with the following minimum requirements:

  • One of the following license classifications:
    • Commercial Class A
    • Non Commercial Class A (FF Restricted)
    • Commercial Class B
    • Non Commercial Class B (FF Restricted)
  • Maintain an “X” endorsement (tank vehicle and hazardous materials endorsement).
  • Maintain an AIR BRAKES endorsement in addition to the “X” endorsement.

Members holding a Commercial Class A or B with an “X” endorsement must complete a TSA background check. . TSA background check must be renewed every 5 years at a cost of $89.25. It can be obtained/renewed at www.hazprints.com or call (877) 429-7746. Members with a FF Restricted DL are not required to obtain TSA clearance.

  • Maintain a current DL-51 (Medical Examination Report) for a commercial license OR DL-546 (Health Questionnaire) for a FF Restricted (non-commercial) license on file with the DMV (good for 2 years).

IMPORTANT NOTE: Make a copy of the original DL-51 or DL-546 and submit the original to the DMV personally. Get a “computer generated receipt” as proof that the DMV accepted the medical form. IF YOU CHOOSE TO MAIL IT, mail the completed medical report at least four weeks before the expiration date. This is to allow enough time for the DMV to process it.

Fax the DMV “computer generated receipt” to In-Service Training Section / Driving Coordinator at (213) 485-8478 so Network Staffing can be updated.

Note: A DL-51A, the wallet size green card (see below), is not proof of a DL-51 on file with the DMV. If a member shows up on the Driver’s License Status Report as having an expired medical, it means the DMV does not have a medical on file and the member needs to provide a valid DL-51 to them.

DRIVERS LICENSE RENEWAL

When notified of a driver’s license renewal by the DMV or to downgrade from a Commercial DL to a FF Restricted DL, you MUST do the following:

· Be prepared to take the following written tests at a DMV office:

o Commercial Class A or B Gen’l Knowledge OR FF Restricted Class A or B FF Gen’l Knowledge

o Air Brakes

o Hazardous Materials

o Tank Vehicle

o Class C Gen’l Knowledge

(You may not have to take all the tests; it is left to the discretion of the DMV as to which tests you will take. You WILL always be required to take the Haz Mat test).

· Prior to leaving the DMV office YOU MUST:

o Double check that the class of license is correct,

o An “X” endorsement is printed on your license

o All the restrictions meet Dept. requirements. (refer to the picture on this page)

UPDATING NETWORK STAFFING

Field personnel can no longer update driver’s license information on Network Staffing. When the need arises to make the necessary updates, contact the DMV Driving Coordinator at In Service Training (213) 485-0866. He will verify through the DMV database that the forms have been received AND processed. If the database does not show the change then Network Staffing will not be updated.
